Staff from Halton-Peel FHTs present findings at AFHTO conference

Staff from several Halton-Peel Family Health Teams (FHTs) recently presented their findings at the AFHTO conference. 

Registered Kinesiologists and Registered Dietitians from Central Brampton FHT, North Peel FHT and Wise Elephant FHTs presented the findings of the implementation of the CHANGE Program in Ontario at the Association of Family Health Team of Ontario Annual Conference in Toronto in September 2019.

Anita Singhmar, Registered Dietitian from North Peel FHT and Ariellia Rodrigues, Registered Dietitian Wise Elephant Family Health Team present a poster on “A collaborative team-based approach to diet and exercise in primary care: Experiences from primary care teams and patients in the CHANGE Program” 

The expansion of the CHANGE Program at Central Brampton, North Peel and Wise Elephant FHTs is partly funded through a grant from Ontario Trillium Foundation.
