CHANGE Program now available in Ontario

We are excited to announce that we have completed training five new clinics in Ontario, and the CHANGE Program will be available to patients with metabolic syndrome:

Northumberland Family Health Team (NFHT) in Cobourg has been offering the program since Spring 2017 and have already enrolled 15 patients as of September. They are excited about the impact the program provides for their patients.

Loyalist Family Health Team in Amherstview has also been busy registering patients for the CHANGE Program since Spring 2017 and have enrolled 10 patients as of September.

LFHT recently announced Carolyn Tereszkowski has joined as the registered dietitian (replacing Maria McGill) - congratulations Carolyn!

We recently completed training three additional clinics and they will begin registering patients for the CHANGE Program in Fall 2017 or early 2018:

Connexion Family Health Team in Ottawa 

Manitouwadge Family Health Team in northern Ontario

Brockton and Area Family Health Team in Walkerton
